 Volunteer Registration Instuctions Minimize

 Thank you immensely for taking the time to volunteer your time to provide an opportunity for hundreds of children to participate in REGION 1046 soccer.

AYSO NATIONAL requires EVERY VOLUNTEER, on an annual basis (that’s every year), to complete a Volunteer Application Form. All “new” Volunteers (those not yet assigned an AYSO ID# and listed as a prior volunteer in EAYSO) must also have the Regional CVPA (Child and Volunteer Protection Advocate) verify, in person, a valid Photo Identification.

Volunteer Application Forms can be completed online:

At WWW.EAYSO.ORG and completing and printing TWO COPIES of the form

New Volunteers must meet in person with the CVPA or other designated AYSO board member, and have a photo ID verified.

Returning Volunteers may sign and date their completed form and mail it to:

WWW.EAYSO.ORG INSTRUCTIONS (ADULT VOLUNTEER)

QUICK INSTRUCTIONS

   1. GO TO WWW.EAYSO.ORG at a computer with a printer.
   2. Follow the instructions after the “First Time” button (unless you've already logged in before.
   3. Print two copies of the application form when prompted. (See #9 below.) Finish.
   4. Bring or mail both signed and dated forms to the region CVPA for photo ID verification. “New” volunteers must submit forms in person to CVPA.

MORE DETAILED EAYSO INSTRUCTIONS

Before filling out the online application, get the names, addresses and phone numbers of 3 references, this is required information, one should be from prior experience with youth sports, and is marked optional but has mandatory fields so if you have no previous youth sports experience, then use any other reference.   There are mandatory fields for Driver’s License number and State ID number; you can use your driver’s license number for both. Be sure to print 2 copies!  The printing option will be shown after you are completely done, don't use screen prints! (your SS# and DL# will not print on the official copy, this is OK)

(Note: At times the EAYSO site loads slowly. Give it time to process requests before requesting again. Also, if your Firewall setting is High, the program may not process properly. Drop protection to Medium while completing the form.)

   1. GO TO WWW.EAYSO.ORG at a computer with a printer.
   2. Follow the instructions for the three choices: 1) First Time; 2) Log In; or 3) Forgot Password.
   3. Follow the prompts to get to “AYSO VOLUNTEERS” button and select it.
   4. Complete name and phone number as requested. Complete fields to obtain a password.
   5. [Page 1 of 7] Follow the prompts to select a Region. Select by zip code or State and City. This part loads slowly, so be patient.
   6. [Page 2 of 7] Complete all of the fields in the Volunteer Application. Red stared fields are mandatory.
   7. [Page 6 of 7] Read the Waiver statement.
   8. [Page 7 of 7] Click on the “I Agree” button if you understand and agree, then type in your name in the signature box and the date in the date box. Then, click “NEXT.”
   9. ***IMPORTANT*** Under Volunteers, click on your name (underlined in blue). This will show you a print preview of your form. At the top right side of the page, in blue, point and click where it says, “Click Here To Print.” Change the number of copies to “2”, then click “Print.” Then go back to the top of the page and click on the “Close Window” button. Not done yet!!
  10. Now click the “NEXT” button, “Next” button, then “FINISHED”!!
  11. Bring or mail both forms signed and dated to the CVPA. New volunteers must submit the form in person to the CVPA and present a VALID PHOTO IDENTIFICATION, matching the information provided on the application.

YOUTH VOLUNTEER INSTRUCTIONS

Youth volunteers may register on-line at WWW.EAYSO.Org, or complete the triplicate Youth Volunteer application Form available from the CVPA or the Referee Administrator. Forms must then be submitted to the CVPA, including PARENT’S SIGNATURE.

NOTE: ALL Youth Volunteers must at all times carry in their possession their copy of the Youth Volunteer Application Form while serving as a volunteer.

SAFE HAVEN ONLINE CLASS

Volunteers may take an online safe haven class (this class is also taught in many referee classes and at least 2x per year in a classroom setting).   The "in-person" class is recommended but for circumstances when that is not possible, AYSO has developed an excellent online class.   To take the class, you must have registered as an AYSO volunteer and that registration must be entered into the national system, it will generate your AYSO volunteer ID at that time.   You can email cvpa@ayso1046.org to inquire about your volunteer ID.   After your volunteer ID is recieved, you can take online classes at   http://www.aysotraining.org/  Be sure to take the Course C001 - Safe Haven for Coaches if you will be a head coach or assistant coach.   Referees, Board Members and Team Parents can take the Safe Haven for Referees as it is a bit shorter.
