AYSO Registration - Fall Season 2008
ON-LINE FALL REGISTRATION NOW OPEN

Cost: $85 per player (early-bird registration prior to August 16)
            Regular registration fee of $100 applies after August 16, 2008.
(Family maximum $255)

This includes 10 weeks of play, player's insurance and a complete uniform: jersey, shorts and socks.
Players will also receive a picture package which includes team and individual pictures.

Two Options for Registration: Walk-up or Mail

Registration is a 2-step process.

FIRST step - register through eAYSO (www.eayso.org). * 

Those of you who previously registered your child through eAYSO will be able to go in, click on “MyeAYSO’ then use the "Player" option.  Click on child’s name and update information. 

 

New players will click on “MyeAYSO” then type ‘1046’ in the region field.  Complete required information.

 

If you experience problems with the online registration system,

please contact 866.588.2976 toll-free in Torrance, CA during normal business hours.

 

* Note:  If you have a returning player from last year, you do not need to register on-line.
 Pre-printed forms will be available during walk-up registration only.

SECOND step - submit your registration package using one of the 2 options below. 

Your registration package includes:

1. TWO printed copies of the registration form signed by a parent/legal guardian,
2. a check (or cash - no credit cards) made payable to AYSO 1046 and,
3. a copy of your child’s birth certificate if they are a new player to our region for review.

Bring to one of the following Walk-up registrations:

Thursday Aug 7^th – Dick's Sporting Goods, Tempe (8550 S. Emerald Dr, Tempe AZ - Next to Ikea)

6:30pm – 8pm:       Open Registration

Saturday, Aug 9^th – Akimel Middle School (2720 E Liberty Lane, Phoenix AZ)

9am – 10:30am:     This time is reserved for our families who volunteered their time during last season.

10:30am – 2pm:     Open Registration

Tuesday Aug 12^th – Sports Authority Ahwatukee (NEW LOCATION * 4820 E. Ray Road, Ahwatukee , AZ 85044)

6:30pm – 8pm:       Open Registration

Thursday Aug 14^th – Sports Authority Chandler Fashion Center (3455 W Frye Rd, Chandler, AZ - Next to Target)

6:30pm – 8pm:       Open Registration

Saturday Aug 16^th – Aprende Middle School (777 N Desert Breeze Blvd E, Chandler, AZ)

9am – 1:00pm:       Open Registration

Remember that it is ONLY by completing these steps that your child is registered - just doing the online part does NOT register your child.

Registration closes August 19, 2008

Registration packet must be given to Registrar by 1:00 pm.
Mail-in registrations must be post-marked by August 14, 2008

After August 19, 2008 – only wait list requests may be accepted.

Due to team balancing and fair play, no coach requests (except own child) will be honored.

We will try to honor one "Buddy Request" but cannot guarantee placement.
